We aim to make a Service User's admission as smooth and as comfortable as possible. Admission should not be about going into care, but simply moving to a new home. The assessment and admission process is by far the most significant process withing the company. It is essential that Service Users are correctly matched to a home where they feel comfortable and at ease.

Your first step as a carer, relative or friend is to contact your local social services department and explain your situation. Each social services department may have slightly different procedures, but the Service User will be assigned a social worker sometimes referred to as a Care Manager. The Care Manager will act as a liason between yourself and Close Care Cymru Ltd. The Care Manager will act proffessionally and will remain unbiased throughout, ensuring the person with learning difficulties recieves the best possible care at all times.

An initial assessment will be carried out prior to the admission of a Service User. This is done by obtaining the following.

  1. Information on any health requirements
  2. Personal interviews / discussions
  3. Reports from proffessionals
  4. Medication requirements
  5. Reports from the Service User's background
  6. Information from current care personnel, nurses, family, advocates or keyworkers.
  7. Any relevant information in order to provide a smooth transition into the home.

 

All information will be gathered and carefully studied in order for the company to fully comprehend the individual's requirements. If possible, an allocated key worker will work with the potential Service User to develop a rapport prior to admission. An individual Care Plan will be formulated as far as physically possible in order to make the Service User's transition to the home as smooth as possible.

The Service User will also be offered trial visits with family or friends should they so wish.  This could consist of informal discussions, meals, activities, viewing of locality including amenities and a viewing of the home.  The Service User will also be offered a chance to 'test drive' the home. This will involve the choice of an overnight stay, or weekend stay in order for them to be satisfied with their environment and to bring forward likes and dislikes. This will ensure that the Service User is matched correctly to the environment and is satisfied with other Service Users within the home.
